1. The subject of the contract is the successive delivery of disposable and reusable medical equipment for endoscopic urological procedures and stereotactic masks divided into two tasks.
Task 1: Disposable and reusable medical equipment for endoscopic urological procedures
Task 2: Stereotactic masks.
2. A detailed description of the subject of the order, assortment and estimated quantities are given in the Assortment and Price Form (Annex 2 to the SWZ) and in the Draft Agreement (Annex 5 to the SWZ).
3. The Ordering Party requires a medical device to be offered in each task.
4. The Contractor provides a 24-month warranty during the warranty period, which is included in the Contractor's remuneration for the subject of the contract (task 1).
5. The Contracting Authority requires the Contractor to provide in the assortment and price form the numbers or catalogue symbols, the name of the manufacturer of the offered assortment and the number of pieces
in the package.
