1. The subject of the contract is the provision of services by specialist staff in the person of integration supervisor and teacher within the framework of project No. RPWP.07.02.02-30-0001 / 18-00, entitled "Dissemination of technologically assisted functional diagnostics and rehabilitation of children and young adults with MPD in the Greater Poland Voivodeship" as part of the Wielkopolska Regional Operational Program for 2014-2020 co-financed from the European Social Fund.

2. A detailed description of the subject of the contract is attached as Annex 2.

3. A detailed training schedule for parents and guardians is provided in Annex 3.

4. The Contractor is obliged to fulfill the contract on the terms and conditions described in the model contract constituting Annexes 6 and 6a.

5. The Awarding Entity allows the submission of partial bids.
