Provision of a Community Specialist Palliative Care Service with potential to increase the scope if agreed between the Parties during the Term to include an End of Life Coordination Centre.

Potential providers should note that the estimated total contract value is based upon the total possible Term allowed (calculated using the 5 year contract Term plus an option to extend for another 2 years).

The Requirement refers to the provision of a Community Specialist Palliative Care Service, characterised by providing generally the following general services, all of which will be further detailed to shortlisted Tendering Organisations in the Invitation to Tender:

1.1.1.Service Management of the Centre

1.1.2.Bereavement Counselling

1.1.3.Support and Advice to Care Home Staff

1.1.4.Patient Support and Advice

1.1.5.Coordination of patient care with other providers

1.1.6.Telephone Advice to Carers and Professionals.
