Community Respiratory Services COPD. North Lincolnshire Clinical Commissioning Group wishes to commission an enhanced respiratory service delivered from community based settings across the area. We would like to notify you we will be seeking suitably qualified and experienced Provider(s) to provide equitable, accessible and inclusive services to address the needs of and support patients. The CCG expects that the enhanced respiratory service will deliver healthcare to our local population covering the following services and conditions: Services to be delivered by the Provider include: — Pulmonary Rehabilitation Service — Self-Management Education — Advice and Support to Primary Care — Early Supported Discharge Scheme for COPD patients — Assessment and Review for Home Oxygen service — TB services (except diagnosis of TB) — Consultant and Nurse Specialist Community Respiratory Clinics — Conditions to be support will include — COPD — Asthma (except ‘difficult-to-control’ Asthma) — Bronchiectasis — Obstructive Sleep Apnoea Providers should be able to demonstrate a track record of delivery of outcomes focussed, specialist health services within the community which offer excellent quality of care and patient experience. Experience of working in partnership with a range of stakeholders and commissioners is also required.
