The subject of the order is the provision of post-warranty service for GE apparatus - Package No. 1 and Philips - Package No. 2, for 36 months for Centrum Onkologii - Instytut im. Maria Skłodowska-Curie in Warsaw.

The subject of the order was specified in detail:

a. in Annex 1.1 to the ToR - Description of the subject of the contract and 1.1A and 1.1B to the ToR - Description of the object of the contract / Price form;

b. in Annex 1.2 to the ToR - Description of the object of the contract and 1.2A - Description of the object of the contract / Price form.

In accordance with art. 29 paragraph 3 a of the Public Procurement Law, the Employer requires employment by the Contractor or subcontractor under an employment contract (for the entire duration of the contract) min. 1 person (service engineer) performing activities in the field of the contract in the manner specified in art. 22 § 1 of the Act of 26.6.1974 - Labor Code - at the latest on the day of signing the contract, the Contractor shall submit to the Ordering Party a declaration of compliance with the abovementioned
