London Clinical Commissioning Group (CCG) commissioners seek applications for the provision of NHS continuing healthcare domiciliary care services. The Any Qualified Provider (AQP) process covers service users eligible for NHS continuing healthcare mental health adult (including dementia); continuing healthcare physical disabilities adult (including frail elderly); and continuing healthcare learning disabilities adult. It also includes end of life care. Service users must be over 18 years old.

Phase 3 of the pan-London NHS CHC domiciliary care AQP contract will be open for applications from 30.4.2018 to 8.6.2018 at https://procontract.due-north.com

Successful providers will be added to a domiciliary care AQP list which is used by London Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare domiciliary care placements. The AQP is the standard route of commissioning for the participating CCGs. The contract will use the full-length NHSE Standard Contract.
