The subject of the present procurement procedure is a framework agreement with a term of three years for the establishment of a digital discharge and transition management.
The clients are recipients of subsidies from the Hospital Future Fund and then obliged by the funding decision to comply with (antitrust) procurement law. As contracting authorities within the meaning of the GWB, some contracting entities are in any case bound by (cartel) procurement law.
Accordingly, it is intended in the present case to procure the services specified in the procurement documents in the name and for the account of the contracting entities listed in Annex 3 to the contract. These become framework agreement partners on the client's side.
Hospitals are obliged to ensure discharge management into follow-up care. The digital discharge and transfer management is primarily intended to relieve the burden on the employees in the hospital who organize the follow-up care / follow-up treatment.
The structured and digital data exchange with the downstream service providers is also to be implemented. The transition from hospital care to further medical, rehabilitative or nursing care is a particularly critical phase in the adequate care for the affected patients.
In particular, the following mandatory criteria must be met:
• Structured data exchange:
A structured exchange of data between service providers and the provision of documents to downstream service providers is made possible. In addition to the classic master data, this concerns, for example, medication intake, advice on nutrition, intolerances, allergies, limitations of physical resilience, necessary control examinations, contact persons in case of complications or nursing questions, etc. Data exchange and document transmission must be based on recognised standards. This is expected to result in an efficient transition process.
• Reporting and search system for downstream processes:
Hospital staff must be able to report the care needs of their patients within a network of outpatient and inpatient care or rehabilitation providers. Furthermore, they must be able to receive timely feedback within a network regarding suitable free capacities and be able to initiate further steps digitally. Above all, this refers to a reservation of the free capacities, which can be booked after consultation with the patient.
