1. The subject of the contract is: "Supply of electricity", for the Independent Public Health Care Team in Brzesko, at 68 Kościuszki Street.

The subject of the contract is the supply of electricity to the Ordering Party's facilities to the following stations: 1) "SZPITAL" station - tariff group B23: - supply of electricity to the transformer station 15/0.4 kV, - connection power Pp = 740 kW, - contractual power Pu = 400 kW, - electricity demand approx. 1 600 000 kWh - annually; 2) Health Centre in Uszvi - tariff group C11: - connection power Pp = 60 kW, - contractual power Pu = 60 kW, - electricity demand approx. 5 000 kWh - annually; 3) Health Center in Wola Dębińska - tariff group C11: connection power Pp = 12 kW, contractual power Pu = 12 kW, electricity demand approx. 10 000 kWh - annually. 3. The estimated amount of electricity for the period of 12 months in total for all tariffs is: 1 615 000,00 kWh.
