The subject of the contract are: Provision of services in the field of collection, transport

and disposal of medical waste from the District Specialist Hospital

in Stalowa Wola to the Ordering Party's registered office in accordance with the offer form - Appendix No. 1, the price form - Appendix No. 2 and the Description of the subject of the contract - Appendix No. 3 and the requirements contained in the SWZ. Other terms of the contract are specified in the contract template constituting Annex 8.
