Description:
Purchase of medical equipment and other medical equipment for the needs of the Orthopaedic Department and the One Day Ward

1. The subject of the contract is the delivery, installation, assembly and commissioning of equipment for the newly built Orthopedic Sub-Department and the One Day Branch in SP ZOZ in Siemiatycze along with the instruction of the Ordering Party's staff in the operation and maintenance of the delivered equipment. 2. A detailed description of the subject of the contract, including the limit requirements for the above-mentioned medical equipment, is attached as Annex 2 - 2c to the SWZ - assortment and price form. Divided into parts (number of parts 4). 3. In view of the above, it is not possible to carry out an on-site visit in order to establish precise measurements relating to the delivery, installation and installation of these items. 4. The Contractor shall be obliged to perform all necessary repairs arising in the places of installation and assembly works and, if necessary, in adjacent premises. 5. The Contractor shall, no later than within 5 days of signing the contract, provide the main contractor for construction works (hereinafter also GWR), selected in a separate procedure for the implementation of construction works related to the construction of the Orthopedic Subdivision and the One Day Branch, information materials in the form of DTR of the offered equipment. In addition to GWR, the Contractor shall provide these documents to the Supervision Inspector. The data of GWR and the Supervision Inspector will be provided to the Contractor in writing. 6. The Contractor is obliged to cooperate fully with GWR in order to properly perform the contract. 7. The Contracting Authority may request the Contractor to provide the documents referred to in point 5 above before signing the contract, but not earlier than 10 days from sending the notification/information on the selection of offers. 8. The offered subject of the contract should be free from physical and legal defects, and after delivery to the Ordering Party, ready for use in accordance with its intended purpose, without the need for the Ordering Party to incur additional financial, organizational and technical outlays beyond those resulting from their normal operation and not being the subject of this order. 9. The Ordering Party allows the possibility of delivering the assortment in stages according to the Ordering Party's needs and at its request. 10. The offered subject of the contract must be admitted to the market and use in health care units in the Republic of Poland in accordance with Polish law and European Union law. 11. Ultimately, the place of delivery of the installation and assembly of equipment is the seat of the Ordering Party - SP ZOZ in Siemiatycze, at 8 Szpitalna Street, rooms indicated by the Ordering Party, after prior arrangement. 12. It is allowed to start and conduct instructions on operation and maintenance for the Ordering Party's personnel at a time other than delivery and assembly of the assortment after prior arrangement with the Ordering Party and obtaining his consent. 13. The Contracting Authority does not reserve the obligation of the Contractor to personally perform key parts of the contract within the scope of the subject of the contract. 14. If in the description of the subject of the contract and other attachments - there is any trademark, patent, standard or origin, it should be assumed that the Ordering Party has provided such a description with an indication of the type and allows the submission of equivalent bids with parameters not worse than those given under the term type. At the same time, we would like to remind you that the Contractor who refers to solutions equivalent to those described by the Contracting Authority is obliged to demonstrate that the subject of the contract offered by him meets the requirements specified by the Contracting Entity.
