The service that is the subject of the contract will be implemented based on:

- The Act of 12.3.2004 on social assistance (Journal of Laws of 2018, item 1508, as amended),

- The Act of 19.8.1994 on the protection of mental health (Journal of Laws of 2017, item 882, as amended),

- Ordinance of the Minister of Social Policy of September 22, 2005 on specialist care services (Journal of Laws of 2005 No. 189, item 1598, as amended).

The subject of the contract are specialist care services for people with mental disorders, provided at the place of residence of the mentee - the city of Ostrów Wielkopolski. Number of hours of specialist care services for people with mental disorders - planned minimum number - 6500 hours, maximum number - 9763 hours

1. Teaching and developing skills necessary for independent living, including:

Developing skills to meet basic life needs and social functioning skills, motivating to activity, treatment and rehabilitation, conducting self-service and social skills trainings and supporting, also in the form of assisting in everyday life activities,

Interventions and help in family life,

Assistance in dealing with official matters,

Support and assistance in obtaining employment,

Help in managing money

2. Care as a support for the treatment process, including:

a) assistance in accessing health services,

b) agreeing and supervising dates of medical visits, diagnostic tests,

c) help in buying or ordering medicines at the pharmacy,

d) supervising the taking of medications and observing possible side effects of their use,

e) in particularly justified cases, change of dressings, assistance in the use of auxiliary agents and medical materials, orthopedic items as well as in maintaining hygiene,

f) assistance in reaching healthcare facilities,

g) assistance in reaching rehabilitation facilities;

3. Physical rehabilitation and improvement of impaired body functions:

a) in accordance with medical recommendations or a specialist in the field of physical rehabilitation or physiotherapy,

b) cooperation with specialists in the field of psychological, pedagogical and educational and therapeutic support aimed at multilateral activation of a person using specialist services,

4. Housing assistance, including:

a) in obtaining housing, negotiating and paying fees,

b) in organizing minor renovations, adaptations, repairs, and eliminating architectural barriers,

c) shaping the right relationship of the person obtaining help with the neighbors and the host of the house,

5. Number of hours of specialist care services for people with mental disorders - planned minimum number - 8,500 hours, maximum number - 12,813 hours.

6. The service should be provided on business days in justified cases also on non-working days (Saturdays, Sundays and public holidays).
