The subject of the contract is the sale and delivery to the ordering party of IT equipment and software as well as installation and launch of software on the supplied equipment along with the service of transferring databases, installing the Oracle environment and the virtualization implementation service according to Appendix No. 2 – assortment and price form.

1. The material scope of the Task includes the modernization of the server room infrastructure in the Main Building of the Holy Family Specialist Hospital SPZOZ in Warsaw at 25 Madalińskiego Street; i.e. purchase of hardware and software along with the configuration of necessary applications, virtualization, migration and backup services.

2. As part of the supplies referred to in paragraph 1, the Ordering Party provides:

1) Purchase of database servers – 2 pcs.;

2) Purchase of application servers – 4 pcs.;

3) Purchase of performance arrays – 2 pcs.;

4) Purchase of SAN switches with necessary cabling and FC inserts – 2 pcs.;

5) Purchase of server software – 8 pcs.;

6) Purchase of database software – 2 pcs.;
