1. The subject of the contract is the supply of medical equipment divided into 4 tasks.
2. Assortment, quantity and price specifications, as well as quality standards relating to all essential features of the subject of the contract, are precisely specified in Appendices No. 2/1 to No. 2/4 to the Terms of Reference, which the Contractor is obliged to complete and attach to the offer.
3. The required technical and functional parameters of the ordered equipment, the required conditions in terms of warranty, service, training are specified in Appendices No. 2/1a to No. 2/4a to the Terms of Reference, which the Contractor is obliged to fill in and attach to the offer.
4. In accordance with the requirement set out in Article 246(2) of the Public Procurement Law, the Contracting Authority, using the price criterion of a weight exceeding 60%, specified in the description of the subject matter of the contract (Appendices No. 2/1 to No. 2/4 to the Terms of Reference) quality requirements relating to at least the main elements constituting the subject matter of the contract.
Task No. 1 Delivery of a 42-channel single-station EEG device1. The assortment, quantity and price specification, as well as the quality standards relating to all essential features of the subject of the contract, are specified in detail in Appendix No. 2/1 to the Terms of Reference, which the Contractor is obliged to fill in and attach to the offer.
2. The required technical and operational parameters of the ordered equipment, the required conditions in terms of warranty, service and training are specified in Appendix No. 2/1a to the Terms of Reference, which the Contractor is obliged to complete and attach to the offer.
Task No. 2 Delivery of a 42-channel two-station stationary EEG machine with EEG video, holter and accessories1. The assortment, quantity and price specification, as well as the quality standards relating to all essential features of the subject of the contract, are specified in detail in Appendix No. 2/2 to the Terms of Reference, which the Contractor is obliged to fill in and attach to the offer.
2. The required technical and functional parameters of the ordered equipment, the required conditions in terms of warranty, service and training are specified in Appendix No. 2/2a to the Terms of Reference, which the Contractor is obliged to fill in and attach to the offer.
Task No. 3 Delivery of the colposcope1. The assortment, quantity and price specification, as well as the quality standards relating to all essential features of the subject of the contract, are specified in detail in Appendix No. 2/3 to the Terms of Reference, which the Contractor is obliged to complete and attach to the offer.

Task No. 4 Delivery of incubators1. The assortment, quantity and price specification, as well as the quality standards relating to all essential features of the subject of the contract, are specified in detail in Appendix No. 2/4 to the Terms of Reference, which the Contractor is obliged to fill in and attach to the offer.
