1. The subject of the order is:

1) lease of a fully automated basic analyzer for immunohematological tests and a back-up analyzer working in microcolumn technology together with a license for software for managing test results and the required instrumentation, as well as connecting to the IT system of the Employer. Currently, the IT system is provided by Asseco Poland SA;

2) lease of devices for manual micromethod, i.e. 4 centrifuges for microcards, 2 incubators, 5 automatic pipettes with adjustable volume;

3) gradual supply of reagents, standard blood cells and all consumables, including system fluids used in immunohematological tests for manual and automatic micromethods.

A detailed description of the subject of the contract is set out in Annex 3 to the ToR.

1. The Awarding Entity requires a deposit of PLN 10,000.00 (say: ten thousand PLN 00/100).

A short opz is included in section II.1.4) Scope of order includes: 1) Installation, commissioning of analyzers and all equipment used in manual micrometrics.

2) Ensuring the correct operation of the devices that are the subject of the contract during the lease, through their servicing, periodic validation, technical review and repair in case of failure.

3) Bidirectional data transmission from the offered analyzers in the scope of registration, ordering tests and sending results, both for blood donors and recipients to the computer system used. 4) Training staff after installing the equipment and providing remote service assistance.

5) Meeting the quality requirements for reagents and consumables necessary to perform the tests and other necessary parameters specified in detail in the ToR.

A detailed description of the subject of the contract is set out in Annex 3 to the ToR.

1) In order to confirm that the offered deliveries meet the requirements specified by the Employer, the Contractor (whose offer will be rated the highest), at the request of the Employer is obliged to submit within a designated, not less than 10 days, the date valid on the day of submitting the statements or documents regarding the offered analyzers , devices for manual micrometers (i.e. centrifuges for microcards, incubators, automatic pipettes with volume control) of reagents and if it concerns standard blood cells, i.e.

a) Manufacturer's declaration of compliance with the essential requirements by the offered medical devices together with the certificate of the notified body, in cooperation with which the conformity assessment procedure of the offered medical devices listed in art. 29 paragraph 5 of the Act of 20.5.2010 on medical devices (Journal of Laws of 2017, item 211, as amended), hereinafter the Medical Devices Act).

b) Notification of the product to the President of the Office for Registration of Medicinal Products for Medical Devices and Biocidal Products (hereinafter the President of URPLWMiPB) or notification to the President of URPLWMiPB about the introduction of the product on the territory of the Republic of Poland in accordance with art. 58 of the Act of 20.5.2010 on medical devices or other document confirming the fulfillment of the obligation arising from art. 58 of the Medical Devices Act. If the Contractor is not subject to the notification / notification obligation (referred to in the previous task), a W statement containing the grounds for the absence of this obligation.

2) In order to confirm that the offered subject of the contract meets the requirements specified by the Employer, the Contractor (whose offer will be rated the highest), at the request of the Employer is required to submit within a designated, not less than 10 days, date valid as at the date of submission:

a) Description of the subject of the contract constituting Annex 3 to the ToR, completed in accordance with its guidelines.

In order to confirm the absence of grounds for exclusion referred to in art. 24 paragraph 1 point 23 of the PPL, W, within 3 days of posting the information referred to in Art. 86 section 5, is obliged to provide Z with a statement on belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23, in accordance with the formula state. Annex 2 to the ToR.
