2.1. The subject of this contract is the successive supply of reagents along with the lease of analyzers for the needs of Analytical Laboratories and the Microbiological Laboratory SP ZOZ of the Municipal Hospital Complex in Chorzów. The subject of the contract includes 9 packages.
Package 1: Lease of a 5 diff blood counts analyzer with the supply of reagents, calibrators, control and additional equipment for the needs of the Analytical Laboratory3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 1: from the date of signing the acceptance protocol (not earlier than 09.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery date at 28 days for package No. 1 – from the date of conclusion of the contract) and in the next maximum 2 working days to train the staff. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 2: Purchase and delivery of reagents with lease of an analyzer for hematological tests – morphology 3 diff for the needs of the Analytical Laboratory, unit at 7 Władysława Truchana Street3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 2: from the date of signing the acceptance protocol (not earlier than 12.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery date at 28 days for package No. 2 – from the date of conclusion of the contract) and in the next maximum 2 working days to train the staff. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 3: Purchase and supply of reagents and necessary consumables, calibrators and test controls with the lease of three automatic analyzers including an IT system3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 3: from the date of signing the acceptance protocol (not earlier than 16.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery time at 28 days for packages No. 3 – from the date of conclusion of the contract) and in the next maximum 2 working days to train the staff. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 4: Purchase and delivery of reagents along with the lease of an analyzer for coagulological tests for the needs of the Analytical Laboratory, unit at 7 Władysława Truchana Street3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 4: from the date of signing the acceptance protocol (not earlier than 26.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery period at 28 days for package No. 4 – from the date of conclusion of the contract) and in the next maximum 2 working days to train the staff. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 5: Purchase and delivery of reagents and test strips with lease of a urinary strip reader for general urine tests for the needs of the Analytical Laboratory3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 5: from the date of signing the acceptance protocol (not earlier than 18.09.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery period at 28 days for packages No. 5 – from the date of conclusion of the contract) and in the next maximum 2 working days staff training. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
3.2. Deliveries will be carried out in accordance with the current needs of the Ordering Party. Delivery time of reagents:
a) Package 5 - in accordance with the declared date in the offer form (maximum delivery date: 7 business days (i.e. from Monday to Friday except public holidays from 8:00 to 14:00) from the date of placing the order,
c) The Contractor will deliver the ordered goods directly to (depending on the assortment):
2) Analytical Laboratory at 7 Władysława Truchana Street – concerns package no. 5;
under the conditions set out in the Draft provisions of the agreement. Orders will be submitted by fax or e-mail only by the Analytical Laboratory at 11 Strzelców Bytomskich Street and 7 Władysława Truchana Street as well as by the Microbiological Laboratory at 11 Strzelców Bytomskich Street.
Package 6: Purchase and delivery of reagents with accessories and lease of a POCT analyzer for diagnostics of critical parameters for the needs of the ZSM Intensive Care Unit3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
- Package No. 6: from the date of signing the acceptance protocol of the devices.
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery time at 28 days for packages No. 6 – from the date of conclusion of the contract) and in the next maximum 2 working days staff training. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 7: Purchase and delivery of reagents with accessories and lease of a POCT analyzer for diagnostics of critical parameters for the needs of the ZSM Intensive Care Unit3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 7: from the date of signing the acceptance protocol.
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery period at 28 days for packages No. 7 – from the date of conclusion of the contract) and in the next maximum 2 working days to train the staff. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 8: Lease of a microbiological analyzer to perform identification and antibiograms, along with the supply of accessories and tests for the needs of the Microbiological Laboratory3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 8: from the date of signing the acceptance protocol (not earlier than 24.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery date at 21 days for package No. 8 – from the date of conclusion of the contract) and in the next maximum 2 working days staff training. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
Package 9: Lease of an EIA+CLIA analyzer for automation of tests along with the delivery of tests for the needs of the Microbiological Laboratory at 11 Strzelców Bytomskich Street3.1. Deadline for the execution of the order:
a) successive deliveries of reagents and accessories - the Ordering Party expects the order to be completed from the date of delivery and installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order:
Package No. 9: from the date of signing the acceptance protocol (not earlier than 24.06.2023).
b) delivery, installation, commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place within the time limit specified by the Contractor in the Offer Form (the Contracting Authority has specified the maximum delivery date at 42 days for package No. 9 – from the date of conclusion of the contract) and in the next maximum 2 working days staff training. Delay in delivery, installation, start-up and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7 section 1 point c) of the Agreement, however, the calculation of the lease rent may start not earlier than from the date of signing the final acceptance protocol of laboratory equipment.
