The service will provide a clear link to primary care from the acute trust(s). The discharge coordinators will work with admitted patients from the selected practices who have little or no formal support. The service aims to cover all 39 GP practices within Greater Huddersfield. The PCDCs will not duplicate the work and involvement of other teams i.e. Virtual Ward, they will compliment it.

The PCDCs will work with patients from their allocated practices, contacting them during the admission and supporting them through and post discharge. Providing a seamless service to the patient and a consistent point of contact.
