Description:
Services of sanitary transport of patients and transport of biological materials, transport of blood and its components.

1. The subject of the contract are services of sanitary transport of patients and transport of biological materials, transport of blood and its components for the Fryderyk Chopin Clinical Provincial Hospital No. 1 in Rzeszów at 2 Szopena Street and at 2 Rycerska Street.

2. A detailed description of the subject of the contract is included in the Description of the subject of the contract (Appendix No. 1 to the SWZ) and the Price and Offer Form (Appendix No. 2 to the SWZ), which are an integral part of the SWZ.

3. The scope of the contract includes the provision of services for the above-mentioned subjects of the contract for a period of 12 months.

2. A detailed description of the subject of the contract is included in the Description of the subject of the contract (Appendix No. 1 to the SWZ) and the Price and Offer Form (Appendix No. 2 to the SWZ), which are an integral part of the SWZ.

3. The scope of the contract includes the provision of services for the above-mentioned subjects of the contract for a period of 12 months.

4. The Ordering Party shall allow solutions equivalent to those described in the Description of the subject of the contract.

6. DEADLINE FOR THE EXECUTION OF THE CONTRACT:

Deadline for the performance of services: for a period of 12 months, counting from the date of conclusion of the contract.

The Ordering Party allows the possibility of changing the term of the contract in the event of non-performance

during the contract period of the entire subject of the contract as a result of the performance of lower orders – the amendment of the contract requires an annex, except that the Ordering Party unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
