The delivery of long term home support services for eligible adults and children/young people. The service will provide personalised care and support to people in their own homes to enable individuals to live independently. The service will also assist carers/parents to maintain their care role. The service is awarded to 7 providers, each aligned to Coventry and Rugby Clinical Commissioning Group GP clusters. 2 of the 7 providers will also cover mental health and learning disabilities city wide (1 provider to each area (either MH or LD)).
