The vision of the Enhanced Community Care programme’s interim ICT solution is to digitally enable clinicians to deliver increased levels of healthcare across general practice, primary care and community-based services. Aims and Objectives ● Support the move to a digitally enabled, ‘paper lite’ community service for clinicians and service users ● Facilitate the capturing and reporting of activity and outputs of ECC Teams as required through the EMT, HSE Board, DoH and Sláintecare Board reporting requirements at senior level ● Enable improved clinical decision-making by multidisciplinary teams by providing real-time access to patient information at the point of care ● The interim ICT solution should support defined and future-developed care pathways and also assist in the future implementation of the ICCMS solution. ● To provide a familiar and consistent user experience nationally, which will enhance the quality of the data inputted and increase the efficiency and effective use of the system. ● Provide management teams within the HSE with sufficient data required to drive future decision-making
