Description:
"Utilization of mixed (unsorted) municipal waste with code 20 03 01, originating from the area of the Głogów Małopolski Commune"

1. The subject of the contract is the implementation of a service for the management of mixed (unsorted) municipal waste with the code 20 03 01 from the commune of Głogów Małopolski.

2. The estimated amount of mixed (unsorted) municipal waste with the code 20 03 01 intended for management during the contract period will be 3 600 Mg.

3. The amount of waste given in item 2 is an estimated value and the actual amount of waste covered by the subject of the contract may differ from the abovementioned quantities provided by the Employer and will not constitute any claims of the Contractor against the Employer. Contractor, based on the above information indicated by the Employer is required to correctly estimate the amount of waste covered by the contract.

4. The Contractor may make a site visit within the service area before submitting the offer (date of the site visit to be agreed upon a written request or by phone).

Contractor, based on the above information indicated by the Employer is required to correctly estimate the amount of waste covered by the contract.

4. The Contractor may make a site visit within the service area before submitting the offer (date of the site visit to be agreed upon a written request or by phone).

5. Conditions for implementation:

1) in the scope of waste delivery:

a) the waste will be delivered to the Contractor by the operator selected by the Employer in a separate procedure,

b) the waste will be weighed on the Contractor's certified weight.

c) the amount of waste is to be each time entered and confirmed in the operator's documents (weighing ticket, road card or other document) for each vehicle.

d) after the end of each month, the Contractor shall provide the Employer electronically with a weight statement of the waste received, and within the time limits specified by law information on how to manage it.

2) in the scope of transferring waste for development:

a) access to the place and unloading area of waste should be hardened so that the operator's vehicles have the possibility of unloading regardless of weather conditions,

b) the place of unloading should be properly secured against access by unauthorized persons,

c) for all damage to the operator's vehicles resulting from the poor technical condition of access roads and maneuvering areas within the installation, the Contractor shall be responsible, in particular he shall be obliged to:

- repair costs,

- replacement of permanently damaged tires;

d) in the event of a breakdown or other reason preventing the acceptance of waste, the Contractor is obliged to immediately notify the Employer,

e) The Contractor undertakes to accept waste on business days during installation working hours. Receiving waste on other days and hours than those specified above requires prior agreement between the parties;

3) in the scope of records and settlement of waste:

a) each amount of waste actually delivered by the operator will be recorded by the Contractor. The records will include: the amount of waste, the registration number of the vehicle delivering the waste, a specific waste code and the date of delivery of the waste,

b) confirmation of receipt of waste will be a waste card signed by an authorized employee of the Contractor,

c) The Contractor shall provide the Employer with all required reports specified in the applicable waste management regulations, on the dates indicated in those regulations;

4) in the field of waste management:

a) waste management will be implemented in accordance with applicable law, in particular the Act of 14.12.2012 on waste, (Journal of Laws of 2019, item 701, as amended), the Act of 13.9.1996 on maintaining cleanliness in municipalities (Journal of Laws of 2018, item 1454, as amended), the Act on environmental protection (consolidated text, Journal of Laws 2018.799, as amended) and implementing regulations.
