Description:

The End of Life Care at home service will provide direct patient care with an appropriately trained Band 2 HCA (or equivalent) in End of Life Care supported by other health and social care professionals as appropriate. To meet the assessed needs of the patient in their home or care home setting, during what is perceived to be the final six weeks of the patient's life (i.e. those that are rapidly deteriorating and therefore eligible for CHC fast track funding).



This notice is a call for competition under the Restricted Procedure requesting Expressions of Interest in the CHC Fast Track Funded End of Life Care (at Home) service being developed by NHS South Worcestershire CCG, NHS Redditch and Bromsgrove CCG, and NHS Wyre Forest CCG (‘the CCGs’).

NHS Midlands and Lancashire CSU Procurement are conducting this Procurement on behalf of NHS South Worcestershire CCG, NHS Redditch and Bromgsgrove CCG and NHS Wyre Forest CCG.

The Worcestershire CCGs are currently reviewing their CHC Fast Track Funded End of Life Services (at Home) covering the whole population of Worcestershire.

The CCGs initial focus is to invite expressions of interest from relevant providers in the market who have current experience of delivering End of Life Care in the community within the clients' home.

Those providers within the market that qualify under the terms of the MOI, to the next stage will receive the Invitation to Tender and will be invited to submit bids for the service.

Interested providers must be able to provide services for patients registered with Worcestershire GP Practices.

The CCGs will run a transparent process, ensuring all bidders (where applicable) are given equal opportunity.

The End of Life Care at Home service will deliver direct patient care by an appropriately trained Band 2 HCA (or equivalent) in End of Life Care supported by other health and social care professionals as appropriate.

The service will meet the assessed needs of the patient as identified in a care plan, in their home, during what is perceived to be the final six weeks of the patient's life (i.e. those that are rapidly deteriorating and therefore eligible for CHC fast track funding).

All staff will be trained via an evidence based End of Life Care program.

The service will provide one primary function:

— To provide End of life, CHC funded fast track health care in peoples own homes according to their assessed need.

The District Nurse that refers to the service will have carried out the appropriate nursing assessment and risk assessments. They will also ensure that the patients' needs are reviewed as per the CHC guidelines, throughout the delivery of the care package and care adjusted accordingly.

The contract term will be for 2 years plus an option to extend for a further 1 year.

The CCGs will be looking to set a fixed price tariff to include day/night service, and all travel.

The CCGs are looking to award up to 3 contracts to cover the 3 CCGs. The view is to have 1 provider per each CCG. Bidders invited to participate in the final ITT will have the ability to bid for more than one CCG.

Important: To express an interest, providers must include an A4 page within the PQQ to explain the organisation's experience in delivering End of Life care services in the last 3 years. Only those organisations with the relevant experience will be considered.
