The subject of the contract is a group life insurance service for employees of the District Hospital in Chrzanów and their family members. As a result of the outcome of this tender procedure, an agreement will be concluded in accordance with Appendix No. 4 to the Terms of Reference. The Contractor shall issue policies specifying the subject and scope of liability, sums insured, amount of premiums, as well as provide the General Terms and Conditions of Insurance applicable in matters not regulated in the Terms of Reference.
