1. The subject of the contract is the performance of the task entitled Expansion and modernization of street lighting in the City and Commune of Osiek and performing the duties of a supervision inspector

Part 1 Extension and modernization of street lighting in the City and Municipality of Osiek:

I. Modernization of road lighting in the Municipality of Osiek

1. Replacement of the following lighting points (according to power division):

- 321 LED road luminaires with a power of 30W,

- 500 50W LED road luminaires,

- 27 road LED luminaires with a power of 60W,

- 78 75W LED road luminaires,

- 150 LED world sources for use in park and garden luminaires,

2. Cleaning and painting corroded lighting booms – 450 pcs.

3. Replacement/installation of cables for lighting fixtures – in booms mounted on power poles of overhead lines – 915 pcs.

4. Replacement of a complete fuse box – 908 pcs.

5. Measurements of insulation resistance of electrical installation, checking automatic power off – 1 076 pcs.

6. Replacement of timers in all lighting cabinets – 68 pcs.

7. Separation of an overhead line supplying lighting fixtures on AsXsn 2x25mm2 with a length of approx. 77 000 m.

8. Removal of cabinets with street lighting control from transformer stations – 57 pcs.

8. Delivery of dedicated geolocation software enabling inventory and ongoing management and monitoring of the condition of lighting infrastructure.

9. Preparation of as-built documentation containing a report on the level of compliance of lighting with applicable regulations and standards and recommendations for the Contracting Authority regarding the adaptation of lighting to applicable standards and regulations.

11. Modernization of some separate street lighting control cabinets – 11 pcs.

12. The use of reactive power compensation systems in all street lighting control cabinets – 68 pcs.

13. Replacement of booms in poor technical condition and to improve the lighting conditions of the road – 115 pcs.

14. The use of programmable power supplies with definable power profiles in 75W luminaires.

II. Development of road lighting in the Municipality of Osiek

1. Hanging luminaires on existing pole stands:

- STR Osiek Stadion at the address Osiek, Sportowa 5 – 40W – 1 pc,

- Osiek LO at the address Osiek, Wspólna 11 – 30W – 1 pc,

- STR Tursko Wielkie 1 on pole no. 15 – 40W – 1 pc,

- STR Tursko Wielkie 1 on pole no. 17 – 40W – 1 pc,

- Tursko Wielkie 28A – 40W – 1 pc.

2. Construction of a street lighting line with the installation of an SOU cabinet (1 pc) on the existing PGE network, Trafo Mucharzew 3 station by the road Dz.No.: 966/2, 966/1, 193.

3. Construction of street lighting on the existing pole together with the SOU cabinet (1 pc) PGE at Dz. no.: 188 Tursko Wielkie.

4. Construction of street lighting of the Osiek road together with the SOU cabinet (1 pc.) Nowa Dz. no.: 12-432.

5. Extension of street lighting at the Osiek road, Ogrodowa Street, Dz. no.: 11-46/8 powered from the transformer station Osieczko 3.

6. Construction of street lighting in Suchowola by the road Dz. no. 15-3006 (from the school) and Dz. no. 15-1169/3 (to the intersection with Dz. No. 15-2599).

7. Extension of the existing street lighting by the road Dz. no.: 13-72/3 in the direction of the railway crossing with a reference to the steel pole no. 10 on dz.nr.: 13-2386.

8. Construction of street lighting at the road Dz. no.: 10-797 at the address Ossala 110A.

9. Addition to the existing line, section to the railway crossing - Trzcianka Górna.

10. Expansion of the street lighting system – Długołęka.

11. Addition of street lighting line in the village - Matiaszów.

Part 2 Performing the function of an investor's supervision inspector in the implementation of the task entitled "Extension and modernization of street lighting in the City and Commune of Osiek":

Performing the function of an investor's supervision inspector in the implementation of the task entitled "Extension and modernization of street lighting in the City and Commune of Osiek"

The Contractor undertakes and ensures that the investor's supervision will be exercised by persons with the rights specified in the provisions of the Construction Law in the specialty:

a) installation in the field of electrical and power networks, installations and equipment.

The scope of the Contractor's obligations is specified in the Construction Law. (consolidated text – Journal of Laws of 2021, item 2351, as amended)

The inspector undertakes to perform the commissioned investor's supervision in accordance with applicable regulations and Polish Standards, principles of technical knowledge.

The duties of the investor's supervision inspector include;

1) Representing the Investor by controlling the compliance of the investment implementation with the project and the regulations and principles of technical knowledge,

2) Checking the quality of performed works and embedded products, in particular preventing the use of defective construction products and not approved for use in construction,

3) Checking and acceptance of works that are subject to cover or disappearing,

4) Participation in tests and technical acceptance of installations, technical devices,

5) Preparation and participation in acceptance activities, confirmation of actually performed works and removal of defects,

6) Activities related to the disclosure and control of removal by the Contractor of defects found during the warranty period, specified in the contract concluded by the Contracting entity with the Contractor

7) Participation in the transfer by the Contracting Authority to the Contractor of all existing documents referred to in the contract for the execution of works.

In addition, the Contracting Authority entrusts the inspector with the following tasks:

1) issuing instructions to the construction manager (confirmed by an entry in the construction log) concerning:

a) remove irregularities or threats,

(b) the performance of tests or tests, including those requiring the discovery of works or covered elements.

c) presentation of expert opinions on the construction works carried out and evidence of admission to the use of construction products and technical equipment.

2) request the construction manager to make corrections or re-perform defective works.

3) suspension of further works in the event that their continuation may cause a threat or cause unacceptable non-compliance with the project or with the building permit.

The supervision inspector shall be obliged to:

a) visiting the investment implementation site with sending a weekly report and participating in construction councils at least once every two weeks

b) presence at construction sites at each additional request of the Ordering Party

c) documenting visits with an entry in the construction log,

d) providing information on the status of works at the request of the Contracting Entity.

The investor's supervision inspector, as a person performing independent technical functions, is responsible for performing these functions in accordance with the regulations and principles of technical knowledge with due diligence in the performance of work and for the proper organization of work, its safety and quality.

All activities related to the execution of the order will be performed by the Inspector personally. Entrusting the performance of part or all of the supervision to a third party requires the written consent of the Ordering Party.

The Contractor shall be fully liable to the Contracting Authority for the works it performs with the help of subcontractors.
