1. The subject of the contract is the Delivery of medical materials, small medical equipment, dressings of specialist first-aid kits, refills for first aid kits and medical equipment.
2. The subject of the contract is divided into the following 5 parts (tasks):
1) Lot No. 1 - Delivery of medical materials,
2) Lot No. 2 – Delivery of small medical equipment,
3) Part No. 3 - Delivery of specialist dressings,
4) Part No. 4 - Delivery of first aid kits and refills for first aid kits,
5) Part No. 5 - Supply of medical equipment.
3. A detailed description of the subject of the contract and the quantitative specification are specified in Annexes 1A, 1B, 1C, 1D and 1E to the SWZ (which are also forms for calculating the proposed offer price) and Appendix 9 – Description of the subject of the contract.
4. The contracting authority shall allow for the submission of partial tenders. The Contractor may submit a bid for one/two/three/four/five or all parts of the subject of the contract.
5. The contracting authority shall provide for the right of option. pursuant to Article 441 of the Public Procurement Law
Supply of medical materials1. The subject of the contract is the supply of medical materials for 34 WOG and for military units and institutions on supply.
2. A detailed description of the subject of the contract and the quantitative specification are specified in Annex 1A (which is also a form for calculating the proposed offer price) and Annex 9 – OPZ.
3. The delivered subject of the order will be brand new, it will be delivered in packaging protecting against quantitative and qualitative changes.
4. The order item is to be free of defects and must not carry traces of use and damage.
5. The Contractor shall deliver the subject of the contract to the place indicated by the Contracting entity by its own transport. Delivery costs, in particular loading and unloading of goods, shall be charged to the Contractor.
6. The delivered goods must come from production not earlier than 2023, but the validity period on the day of delivery may not be less than 80% of the total period of validity.
Supply of small medical equipment1. The subject of the contract is the Supply of small medical equipment for 34 WOG and for military units and institutions in supply.
2. A detailed description of the subject of the contract and the quantitative specification are specified in Annex 1B (which is also a form for calculating the proposed offer price) and Annex 9 – OPZ.
3. The delivered subject of the order will be brand new, it will be delivered in packaging protecting against quantitative and qualitative changes.
4. The order item is to be free of defects and must not carry traces of use and damage.
5. The Contractor shall deliver the subject of the contract to the place indicated by the Contracting entity by its own transport. Delivery costs, in particular loading and unloading of goods, shall be charged to the Contractor.
6. The delivered goods must come from production not earlier than 2023, but the validity period on the day of delivery may not be less than 80% of the total period of validity.
7. The Contractor shall grant a minimum of 2 years of guarantee for the delivered subject of the contract counting from the date of receipt of the subject of the contract by the Contracting entity, with the proviso that the warranty period granted by the Contractor will not be shorter than the warranty period granted by the manufacturer of the subject of the contract
Supply of specialist dressings1. The subject of the contract is the Supply of specialist dressings for 34 WOG and for military units and institutions being supplied.
2. A detailed description of the subject of the contract and the quantitative specification are specified in Annex 1C (which is also a form for calculating the proposed offer price) and Annex 9 – OPZ.
3. The delivered subject of the order will be brand new, it will be delivered in packaging protecting against quantitative and qualitative changes.
4. The order item is to be free of defects and must not carry traces of use and damage.
