The subject of the contract are accident insurance services for the benefit of Entities, i.e. for organizational units subordinate to or supervised by the Minister of Justice.
Group accident insurance for convicts performing unpaid, controlled work for social purposes and socially useful work1. The subject of the contract is the provision by the Insurer of the service of group accident insurance for convicts performing unpaid, controlled work for social purposes and socially useful work in accordance with the provisions of the regulation issued on the basis of Article 56a § 2 of the Act of 6 June 1997 – the Executive Penal Code (i.e. Journal of Laws of 2023, item 127, as amended), including in accordance with the provisions of the Regulation of the Minister of Justice of 27 May 2010 on the procedure for concluding accident insurance contracts by eligible entities for convicts performing unpaid, controlled work for social purposes and socially useful work (Journal of Laws of 2010, No. 98, item 632, as amended), during the period of validity of this Regulation.
2. A detailed description of the subject matter of the contract is set out in:
a) the template of the Agreement constituting Appendix No. 5a to the Terms of Reference,
b) Appendix No. 1 to the Model Agreement "Description of the Subject of the Contract" constituting Appendices No. 2a to the Terms of Reference;
3. The Contracting Authority, acting pursuant to Article 95(1) of the Public Procurement Law, requires the Contractor or Subcontractor to employ on the basis of an employment relationship all persons who will be responsible for handling the Agreement and will perform activities consisting of: ongoing contact with the Policyholder, the Insured or a broker (correspondence under the Agreement and telephone contacts in matters related to the Agreement), issuing insurance documents, settling the Agreement, confirming the scope of the Insurer's liability, confirming the insurance cover, informing about the status of insurance premium settlements, providing information or explanations related to the performance of the Agreement, on the terms and to the extent specified in the Model Agreement constituting Appendix No. 5a to the Terms of Reference.
4. The meaning of the terms used in the Terms of Use is the same as the definitions adopted in the Model Agreement constituting Appendix No. 5a to the Terms of Reference and the Description of the subject matter of the contract constituting Appendix No. 2a to the Terms of Reference.
5. The list of units for which the proceedings are carried out shall be Annex 8 to the SWZ.

a) loss ratios, are included in Appendix No. 10 to the Terms of Reference.
b) the number of insured persons has been indicated in Appendix 9a to the Terms of Reference, the "Cost Calculation" Form.
Group Accident Insurance for Social Work Minors1. The subject of the contract is the provision by the Insurer of the service of group accident insurance for minors performing social work in accordance with the provisions of the Regulation issued on the basis of Article 152(5) of the Act of 9 June 2022 on the Support and Rehabilitation of Juveniles (Journal of Laws of 2022, item 1700, as amended), including in accordance with the provisions of the Regulation of the Minister of Justice of 31 August 2022 on the procedure for concluding an insurance contract for accidents of minors performing social work by eligible entities (Journal of Laws of 2022, item 1888, as amended) during the period of validity of this regulation.
2. A detailed description of the subject matter of the contract is set out in:
a) the template of the Agreement constituting Appendix No. 5b to the Terms of Reference,
b) Appendix No. 1 to the Model Agreement "Description of the Subject of the Contract" constituting Appendix No. 2b to the Terms of Reference;

The subject of the contract is the provision by the Insurer of the service of group accident insurance for persons referred or placed in Correctional Institutions, Juvenile Shelters, Correctional Institutions and Shelters for Juveniles, in District Educational Centers and Probation Centers, as well as accident insurance and civil liability insurance for persons performing work or activities in the Centers Curatorial Committees.

b) Appendix No. 1 to the Model Agreement "Description of the Subject of the Contract" constituting Appendix No. 2c to the Terms of Reference;
