1. The object of the order is:

a) Supply of medical devices to the paediatric profile electrophysiological system specified in Annex 3 to the Terms of Reference.

b) Lease of an electroanatomical system with equipment for the UCC, in accordance with Appendix No. 4 to the Terms of Reference.

2. The products listed in point 1a must meet the requirements set out in Appendix 3 to the Terms of Reference. Failure to meet at least one of the requirements set out in Appendix 3 to the Terms of Reference will result in rejection of the offer.

3. The apparatus mentioned in point 1b must meet the parameters specified in Appendix No. 4 to the Terms of Reference. Failure to meet at least one of the threshold parameters specified in Appendix 4 to the Terms of Reference will result in rejection of the offer.

4. The Contracting Authority requires that the offered assortment meets the requirements for medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974) and in executive acts issued on the basis of this Act
