The subject of the contract is a service consisting in life and accident insurance for employees and family members of employees of the Świętokrzyskie Psychiatry Center in Morawica.

1) A detailed description of the subject of the contract is included in Annex 1, Annex 1A and 1B to the Specification of Essential Terms of the Contract (SIWZ).

2) The insurance will be available in two variants differing only in the amount of the premium and the amount of benefits.

3) The monthly premium for one person may not be higher than PLN 50.00 in option I and PLN 65.00 in option II.

4) The Employer requires employment on the basis of an employment contract by the Contractor or subcontractor min. 3 persons performing activities during the performance of the contract consisting in booking the impact of the insurance premium for the period resulting from the contract and paying compensation for damage to the Employer in the manner specified in art. 22 § 1 of the Act of 26.6.1974 - Labor Code (consolidated text (Journal of Laws of 2018, item 108).

5) The Employer has the right to inspect the Contractor in terms of compliance with the requirements set out in item 4. At the request of the Employer within a time limit indicated by the Employer not less than 5 working days, the Contractor undertakes to submit for inspection a statement confirming that the employees referred to in item 1 are employed under an employment contract.

6) The Contractor's failure to submit the documents referred to in item 5 will be treated as failure to fulfill the obligation specified in the ToR and Art. 29 paragraph 3a of the Act.
