The Ordering Party requests the payment of the following:

Task No. 1 - the amount of the deposit is PLN 400.00

Task No. 2 - the amount of the deposit is PLN 400.00

Task No. 3 - the amount of the deposit is PLN 240.00

Task No. 4 - the amount of the deposit is PLN 340.00

Task No. 5 - the amount of the deposit is PLN 1,500.00

Task No. 6 - the amount of the deposit is PLN 1,400.00

Task No. 7 - the amount of the deposit pln 1,200.00

Task No. 8 - the amount of the deposit is PLN 1,390.00

Task No. 9 - the amount of the deposit is PLN 40.00

Task No. 8 - the amount of the deposit is PLN 240.00

1. Wadium shall be lodged before the deadline for submitting tenders.

2. Vadium may be brought in one or more of the following forms:

1) money;

2) bank guarantees;

3) insurance guarantees;

4) the surety provided by the entities referred to in Article 6b (5) (2) of the Law of 9 November 2000 on the establishment of the Polish Enterprise Development Agency (Journal of Laws of 2020, item 299).

3. Vadium in the form of money should be made by transfer to a bank account with the number:

The Contracting Authority shall allow a defect to be lodged on one document for all tasks, and the tasks and corresponding quotas shall be listed on the document.

NOTE: The deadline for the payment of the defect in the form of money will be the time limit for the recognition of the Ordering Party's account.

4. The vadium brought in the form of a guarantee or surety referred to in point 2 (4) shall be made as the original warranty or surety in electronic form and meet at least the following requirements:

1. It must include responsibility for all cases resulting in loss of defect by the Contractor set out in the Law of the PZP;

2) from its content should be clearly the undertaking of the guarantor to pay the entire amount of the wad;

3) should be irrevocable and unconditional and payable at first request;

4) the term for the surety or warranty shall not be less than the time limit for tying the tender (subject to the first day of tying the offer is the date of submission of tenders);

5. The contents of the surety or warranty should include the name and number of the proceedings concerned;

6) the beneficiary of the surety or guarantee is:...

7) in the case of Contractors jointly applying for the award of a contract (Article 58 of the PZP), the Contracting Authority shall require that the surety or warranty shall cover by its content (i.e. obligated for surety or warranty) of all Contractors jointly applying for the award of the contract or to its content has resulted in securing the offer of Contractors jointly applying for the award of the contract (consortium);

5. Offer by a contractor who has not brought the defect, or brought the defect incorrectly or has not maintained the defect continuously until the deadline for binding the tender or applied for reimbursement of the defecium in the case referred to in Article 98 (2) (3) of the PZP is rejected.

6. The contracting authority shall reimburse the defect on the principles set out in Article 98 (1) to 5 of the Pzp Act.

7. The Contracting Authority shall retain the wadium with interest in the cases referred to in Article 98 (6) of the Pzp Act.

The Contracting Authority provides for the use of the reverse procedure referred to in Article 139(1) of the Act:

1) The Contracting Authority may first examine and evaluate the tenders, and then make a subjective qualification of the Contractor whose offer has been rated the highest in terms of the lack of grounds for exclusion and meeting the conditions for participation in the procedure.

2) The Contractor is not obliged to submit together with the offer the statement referred to in Article 125(1) of the JEDZ. The contracting authority will request this statement from the contractor whose offer has been rated the highest together with subjective means of proof.

3) If there are grounds for exclusion in relation to the Contractor referred to in point 1, the Contractor does not meet the conditions for participation in the procedure, does not submit subjective means of proof or statement referred to in Article 125(1), confirming the lack of grounds for exclusion or meeting the conditions for participation in the procedure, the Ordering Party shall re-examine and evaluate the offers of other Contractors, and then make a subjective qualification of the Contractor, whose offer was rated the highest in terms of the lack of grounds for exclusion and meeting the conditions for participation in the procedure.

4) The Contracting Authority shall continue the procedure of re-examination and evaluation of tenders referred to in point 3 in relation to the tenders of Contractors remaining in the procedure, and then shall make a subjective qualification of the Contractor whose offer has been rated the highest in terms of the lack of grounds for exclusion and meeting the conditions for participation in the procedure, until the most advantageous offer is selected or the procurement procedure is cancelled.

1. In order to confirm the absence of grounds for excluding the Contractor from participation in the public procurement procedure, the Contracting Authority shall request (upon request) from the Contractor, which will be assessed the highest, the following subjective means of proof:

1) Information from the National Criminal Register in the scope of:

a) Article 108(1)(1) and (2) of the Public Procurement Law Act of 11 September 2019, hereinafter referred to as the "Act",

b) Article 108(1)(4) of the Act on the prohibition of applying for a public contract as a penal measure,

1. The Contractor shall be excluded from the procurement procedure, subject to Article 110 (2) of the Pzp:

1.1 Being a natural person who is legitimately convicted of a crime:

(a) participation in an organised criminal group or a union aimed at committing an offence or tax offence referred to in Article 258 of the Criminal Code;

(b) trafficking in human beings referred to in Article 189a of the Criminal Code;

(h) referred to in Article 9 (1) and (3) or Article 10 of the Act of 15 June 2012 on the effects of entrusting the performance of work to foreigners residing in defiance of regulations in the territory of the Republic of Poland,

— or for an appropriate prohibited act specified in the provisions of foreign law.

1.2 If a sitting member of its management or supervisory body, a partner of the company

in a general or partnership company or a general partner in a limited partnership or a limited joint-stock partnership or a proxy has been convicted of the offence referred to in point 1.1;

1.3 For whom a final court judgment or final administrative decision has been issued

In order to confirm the lack of grounds for exclusion from Article 5K of Regulation 833 /2014 and Article 7(1) of the Act on special solutions for counteracting the promotion of aggression against Ukraine and for the protection of national security (Journal of Laws of 2022, item 835), the contractor shall submit its own statement (self-statement regarding the grounds for exclusion from the proceedings).
