Description:

1. The subject of the contract is group life and health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune. 2. The order has been divided into 2 parts, i.e.

2.1. Part I:

Package I - Group life insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune.

2.2. Part II:

Package II - Group health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune.

A detailed description of the subject of the contract is provided in Annex 1 to the ToR.

3. In these proceedings partial offers may be submitted. Each Contractor may submit an offer for both parts I and II or for parts I or II. Part I covers Package I, Part II covers Package II.

Part I - Package I - Group life insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune

The subject of the contract is group life and health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune, i.e.

- Municipal Board of Municipal Facilities; ul. Wolności 1a; 46-200 Kluczbork,

- Museum of Jan Dzierżona,

- Municipal and Communal Public Library in Kluczbork,

- Environmental Self-Help House,

- Kluczborski Culture Center,

- Center of sport and recreation,

- Social welfare Center,

- Education Administration in Kluczbork,

- Municipal Primary Music School of Ignacy J. Paderewski in Kluczbork,

- Public Kindergarten No. 1 in Kluczbork,

- Public Kindergarten No. 2 in Kluczbork,

- Public Kindergarten No. 5 in Kluczbork,

- Public Kindergarten No. 7 in Kluczbork,

- Public Kindergarten No. 8 in Kluczbork,

- Public Kindergarten in Bogacica,

- Primary School No. 1 in Kluczbork,

- Primary School No. 2 in Kluczbork,

2. The order was divided into 2 lots, ie:

2.1. Part I:

Package I - Group life insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune.

2.2. Part II:

Package II - Group health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune.

A detailed description of the subject of the contract is provided in Annex 1 to the ToR.

3. The Employer does not guarantee that all employed employees and family members of the Employer's employees will join group life and health insurance.

4. There may be changes in the number of insured persons during the contract period.

5. If an employee or a family member of an employee resigns from insurance, the Ordering Party is not obliged to pay an insurance premium for them.

6. Contractors applying for the award of the contract in these proceedings must meet the conditions set out in the Act of 11.9.2015 on insurance and reinsurance activity (consolidated text of the Journal of Laws of 2017, item 1170), and if they began activities before the entry into force of the Act of 28.7.1990 on insurance activities (Journal of Laws No. 59, item 44 as amended) should have a certificate of the Minister of Finance confirming that they have permission to carry out insurance activities.

8. In these proceedings partial offers may be submitted.

Each Contractor may submit an offer for both parts I and II or for parts I or II.

Part I covers Package I, Part II covers Package II.

9. In these proceedings, variants may not be submitted.

10. The Contractor shall indicate in the offer the parts of the contract which it intends to subcontract or the name (company) of the subcontractors, on whose resources the Contractor relies on the principles set out in art. 22a of the PPL, in order to demonstrate compliance with the conditions for participation in the proceedings referred to in art. 22 paragraph 1 Public Procurement Law

Part II - Package II - Group health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune

1. The subject of the contract is group life and health insurance for employees and family members of employees employed in organizational units of the Kluczbork Commune, i.e.
