The subject of the contract is the provision of medical (sanitary) transport services for the Grochowski Hospital, the Praga Hospital and the Warsaw Southern Hospital.
The method of execution will be the same for each part of the order. The subject of the order will be provided 7 days a week around the clock.
The Contracting Authority allows for the submission of partial offers divided into 4 parts of the contract according to the following division:
1) Part of the contract No. 1 / Task No. 1 / Package No. 1 – transport of S with a doctor - specialist team,
2) Part of the contract No. 2 / Task No. 2 / Package No. 2 – transport of T with a paramedic,
3) Part of the contract No. 3 / Task No. 3 / Package No. 3 – transport P - basic unit,
4) Part of the contract No 4 / Task No 4 / Package No 4 – Transport for the needs of Night and Holiday Health Care / Night Medical Assistance
Part of the contract No. 1 / Task No. 1 / Package No. 1 – transport of S with a doctor - specialist teamThe subject of the contract is the performance by the Contractor of transport services with a team S with a doctor (specialist team), also known as "transport", "service" and "transport with a doctor". throughout the day on all days of the week.
Part of the contract No. 2 / Task No. 2 / Package No. 2 – transport of T with a paramedicThe subject of the contract is the performance by the Contractor of transport services with a T-team with a paramedic, also known as "transport", "service" and "transport with a paramedic", throughout the day on all days of the week
Contract part No. 3 / Task No. 3 / Package No. 3 – transport P - basic assemblyThe subject of the contract is the performance by the Contractor of transport services with the basic set P, also known as "transport", "service" and "basic transport" throughout the day on all days of the week
Part of the contract No 4 / Task No 4 / Package No 4 – Transport for the needs of Night and Holiday Health Care / Night Medical AssistanceThe subject of the contract is the performance by the Contractor of transport services for people (nurses and/or doctors) providing night and holiday health care services at the place of residence or stay of the beneficiary in the area of protection of more than 50 thousand beneficiaries in the supplemented scope. The transport includes the arrival of the vehicle and the transport of the nurse or doctor from the Ordering Party's headquarters (Grenadierów 51/59, 04-073 Warsaw) to the patient, waiting for the nurse or doctor and returning with the nurse or doctor to the Ordering Party's headquarters. The transport will be carried out in Warsaw
