The subject of the contract are successive deliveries of hydrogen peroxide to the leachate pre-treatment plant, for a period of 6 months from the date of signing in the amount of 7.7 Mg (6.94 m3) but not less than 6.0 Mg (5.41 m3) (minimum benefit).

The subject of the contract are successive deliveries of hydrogen peroxide to the leachate pre-treatment installation, for a period of 6 months from the date of signing on the terms described below:

1) Hydrogen peroxide 35% (% by mass) technical,

a) Total amount of hydrogen peroxide solution 7,7 Mg (6,94 m3), but not less than 6,0 Mg (5,41 m3) (minimum benefit)

2) Delivery time 48 hours from placing the order, excluding Saturdays, Sundays and public holidays. The maximum time of handling hydrogen peroxide at the place of performance is 2 (two) hours,

3) Unit orders will be sent by e-mail,

4) Frequency of deliveries at least once every 4 weeks; in the event of a failure or downtime in the operation of the pre-treatment plant, the time between individual deliveries may be extended,

5) Single transport of hydrogen peroxide of at least 500 dm3 (575 kg). The maximum amount of hydrogen peroxide will be 2 m3 (2300 kg)

6) Hydrogen peroxide will be supplied in IBC containers with a volume of 1 m3, or will be poured by pump to the perhydrol dosing station into a tank with a capacity of 1 m3.

7) Transport shall be provided by the Contractor.

8) Transport at the plant will be carried out in accordance with ADR regulations.

9) Unloading of hydrogen peroxide in the place indicated by the Contracting Authority shall be ensured by the Contractor.

Unloading will take place from 8:00 a.m. to 2:00 p.m. (except for Saturdays, Sundays and public holidays), in accordance with ADR regulations and the safety data sheet. Unloading of hydrogen peroxide must take place in a two-person team, which is provided by the Contractor (Supplier).

11) For each unit delivery, the Contractor (Supplier) shall attach a document of issue and a certificate of quality of hydrogen peroxide.

12. The hydrogen peroxide release document shall contain in particular the following information:

(a) the date of issue of the document,

(b) the name of the hydrogen peroxide producer,

(c) the place of reception of hydrogen peroxide,

(d) date and time of receipt of hydrogen peroxide,

(e) the quantity of hydrogen peroxide delivered, expressed in kg,

(f) the registration number of the vehicle supplying the hydrogen peroxide.

(g) the signatures of the transmitting and receiving person.

13) The price to be paid by the Contracting entity to the Contractor for each unit delivery shall be determined as the product of the net rate per kilogram of hydrogen peroxide indicated in the offer and the weight of hydrogen peroxide delivered in a given delivery, indicated on the issue document.

Attention:

At the time of execution of the contract, the Contractor is obliged to comply with health and safety regulations and regulations applicable at the place of fulfilment of the provision, among others, requirements related to the PN-EN ISO 14001:2015 environmental management system, which are available in the office of the Contracting Authority on the territory of his bet.

The awarding entity does not provide for reimbursement of the costs of participation in the procedure.
