1. The subject of the contract is the delivery of pacemakers and cardioverter-defibrillators together with accessories and appropriate equipment, hereinafter referred to as the Goods or Device.

A detailed description of the subject of the contract, specifying parameters, technical and functional requirements and quantity is attached as Annex 1 to the SWZ – Detailed Offer Form (for individual packages). This description should be read together with possible changes in the content of the specification, e.g. as a result of answers to Contractors' inquiries.

3. Draft agreements constituting Annex 8A to the SWZ (Packages No. 1 and 2), Annex 8B to the SWZ (Package No. 3) are an integral part of this documentation and the provisions contained therein are treated as conditions for awarding the contract.
