Purchase and delivery of infusion fluids for medical entities of the Podkarpackie Voivodeship.

1. The subject of the contract is the purchase and supply of infusion fluids to the entities listed in

section I.1 of the announcement (except for the entity: Podkarpackie Medical Center in Rzeszów SPZOZ). A detailed description of the subject of the contract is included in Annex 4 to the Terms of Contract.

2. The complete offer shall be made (obligatory):

1. The offer is prepared in accordance with the template of Annex 1 to the SWZ. Offer form.

2. Completed Offer and price form in accordance with the template constituting Annex 4 to the SWZ.

3. Declaration in the form of an ESD according to Appendix No. 2 to the SWZ;

4. Statement, according to Annex 7 to the SWZ – if applicable;

5. Declaration according to Annex 8 to the SWZ.

6. Power of attorney or other document confirming the authorization to represent the contractor.

7. Means of proof in question: Statements (contained in the form Offer form points 3.1-3. Appendix No. 1 to

SWZ if applicable)
