                        Adaptation of existing GESUT databases and creation of an initial GESUT database 
 The subject of the order is 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 

 Imielno 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 1 - Imielno 

 Małogoszcz - a rural area 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 2 - Małogoszcz - rural area 

 Naglowice 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 3 - Nagłowice

 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 4 - Oksa 

 Sędziszów - a city, Sędziszów - a rural area 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 5 - Sędziszów - a city, Sędziszów - a rural area 

 Słupia (Jędrzejowska) 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 6 - Słupia (Jędrzejowska) 

 Sobków 
 1. Adaptation of the existing GESUT poviat database to compliance with the current GESUT conceptual model and creation of the initial GESUT database in accordance with the GESUT conceptual data model, as defined in the Regulation of the Minister of Administration and Digitization of October 21, 2015 regarding the GESUT poviat database and the national GESUT database in selected registration units, specified in Annex 1 to the OPZ. 
 2. Introduction to the Staroste's ICT system of the initial GESUT database created by the Contractor. 
 Task 7 - Sobków
