1. The subject of the contract is the implementation of the task entitled: "Modernization of the IT infrastructure of the server room of the Nowowiejski Hospital in Warsaw". 2. The implementation of the task is covered by co-financing from a grant from the Mazowieckie Voivodeship, based on the concluded Agreement No. W/UMWM-UU/NW/2025/2023 of 28.04.2023. 3. The material scope of the Task includes the modernization of the server room infrastructure in the Main Building of the Nowowiejski Hospital in Warsaw at 27 Nowowiejski Street; i.e. purchase of hardware and software along with the configuration of necessary applications, migration service of the entire system environment for keeping AMMS medical records and the Infomedica HR and payroll system, as well as purchase, installation and configuration of Active Directory.

As part of the deliveries referred to in paragraph 3, the Ordering Party provides: 1) Purchase of virtualization servers – 3 pcs.; 2) Purchase of database servers – 2 pcs.; 3) Purchase of a backup server – 1 pc.; 4) Purchase of Fibre Channel switches – 2 pcs.; 5) Purchase of NAS server – 1 pc.; 6) Purchase of disk arrays – 2 pcs.; 7) Purchase of switches for the core of the network – 2 pcs.; 8) Purchase of virtualization software – 1 set; 9) Purchase of server systems with access licenses – 1 set. 5. Detailed conditions, description of the material scope and manner of performance of the contract are specified in Annex 2 to the SWZ (including Annex 2.1 – detailed description of the subject of the contract), containing minimum requirements relating to the subject of the contract.
