Description:

Medway Council and NHS Medway Clinical Commissioning Group (CCG) are seeking to recommission Community Children's Health Services.

This procurement is an innovative and progressive integration of a host of child health services across Medway jointly commissioned by Medway Council and NHS Medway CCG.

In Medway, we believe that community children's health services should focus on supporting children and families to achieve the outcomes that are most important to them.

We are aiming to build a modern and responsive system of children's community health services that retains families at the centre of interventions, that links well across health and social care disciplines as well as the wider children's workforce, and a service where practitioners are enabled and empowered to respond to individual needs.

There will be 2 lots within this procurement which are detailed within this notice and accompanying descriptive documents.


Healthy Child Programme

Services in Lot 1 will be focused on delivering the Healthy Child Programme (HCP) in an integrated and seamless manner across the whole 0-19 age range.

The key services included in this specification are:

— Integrated Public Health Nursing 0-19 (currently Health Visiting and School Nursing) to lead delivery of the Healthy Child Programme to families including fathers. (The structure and any age related breakdown within this area will be determined through the competitive dialogue process).

— Vulnerable Parents Pathway, Safeguarding and Specialist Health Visitors.

— National Child Measurement Programme (NCMP) in both Yr. R and Yr6.

— Infant Feeding Coordinator.

— Oral Health promotion across the whole 0-19 age range.

The provider will lead, deliver and coordinate in partnership with other agencies the HCP Programme. All children, young people and their families will receive a comprehensive flexible needs-led offer in line with the hard outcomes detailed within this specification and underpinned by the principles and vision of an integrated community children's health service in Medway.


Specialist Community Health Services

Services require in Lot 2 will be for children and young people aged 0-19 that have health and therapeutic needs requiring services additional to those that are provided in Lot 1.

In addition to appropriate staff that will be employed to deliver community children's health services to the following groups of children, there will be a requirement for the following functions to be undertaken, in order to facilitate some key elements of delivery:

i. Coordination of children's community health services pathways for children and young people requiring multidisciplinary support. This function is to be undertaken at the Snapdragons Centre.

ii. Children's safeguarding advice as a part of the Multi Agency Safeguarding Hub (MASH) on a full time basis. This function is to be undertaken at the MASH site where this exists.

iii. Development and coordination of local clinical networks, for example clinical networks for children and young people with neuro-developmental conditions.
