The subject of the contract is the supply of knee joint endoprostheses and hip joint endoprostheses for the Cardinal Stefan Wyszyński Provincial Hospital in Łomża, hereinafter referred to as the Ordering Party, in quantities, assortment and with the required parameters specified in Appendix No. 5 to the Terms of Reference.

The conditions for the fulfillment of the contract are set out in the template of Annex 6 to the SWZ.

The Order is divided into lots, hereinafter referred to as Packages. Number of parts (Packages) – 2.

- Part (PACKAGE) 1 – Knee joint endoprostheses;

- Part (PACKAGE) 2 – Hip endoprostheses.
