The Norfolk Continuing Care Partnership is an operational unit of NHS Norwich Clinical Commissioning Group, providing services to NHS Norwich, NHS West Norfolk, NHS South Norfolk and NHS North Norfolk Clinical Commissioning Groups.
NHS Arden and Greater East Midlands Commissioning Support Unit (NHS AGCSU) on behalf of The Norfolk Continuing Care Partnership is looking for a fast track CHC service involving purchasing nursing homes beds and domiciliary care rounds for the following lots:
Lot 1: 1 x Care Home Bed, West Norfolk,
Lot 2: 1 x Care Home Bed, North Norfolk,
Lot 3-6: 4 x Care Home Beds, Norwich (maximum two beds per home),
Lot 7-8: 2 x Care Home Beds, South Norfolk (maximum one bed per home) (Note: Commissioners require the homes in this location to be within 18 miles, by road, of the Norfolk and Norwich University Hospital).
Care rounds:
(Note: maximum one lot per provider)
Lot 9: 2 x Care Rounds, North Norfolk,
Lot 10: 1 x Care Round, Norwich,
Lot 11: 1 x Care Round, South Norfolk.
The requirement is to deliver care within a Residential Care Home with Nursing (referred to as a Nursing Home) in West Norfolk for those patients who have been assessed as eligible for NHS Fast Track funded care. The service will be delivered to a single patient at a time.
The requirement is to deliver care within a Residential Care Home with Nursing (referred to as a Nursing Home) in North Norfolk for those patients who have been assessed as eligible for NHS Fast Track funded care. The service will be delivered to a single patient at a time.
The requirement is to deliver care within a Residential Care Home with Nursing (referred to as a Nursing Home) in Norwich for those patients who have been assessed as eligible for NHS Fast Track funded care. The requirement is for four beds, a provider may tender a maximum of two beds in a single home.
The requirement is to deliver care within a Residential Care Home with Nursing (referred to as a Nursing Home) in South Norfolk for those patients who have been assessed as eligible for NHS Fast Track funded care. The requirement is for two beds, a provider may tender a maximum of a single bed in a single home. Commissioners require the homes in this location to be within 18 miles, by road, of the Norfolk and Norwich University Hospital.
The requirement is to deliver two care rounds in North Norfolk for those patients who have been assessed as eligible for NHS Fast Track funded care.
The requirement is to deliver a care round in Norwich for those patients who have been assessed as eligible for NHS Fast Track funded care.
The requirement is to deliver a care round in South Norfolk for those patients who have been assessed as eligible for NHS Fast Track funded care.
