1. The subject of the contract are the deliveries of Irinotecani hydrochloridum trihydricum and Vedolizumabum, specified in the parts for the Fryderyk Chopin Clinical Provincial Hospital No. 1 in Rzeszów.

2. Detailed description of the object of the contract, requirements of the Ordering Party, information on the acceptance of other quantities in the packaging, are specified in the Price and Tender Forms — Annex No 1 for individual parts, which are an integral part of the SWZ.

3.The Ordering Party requires offer items of the order that meet the requirements described in SWZ.

4. The scope of the order includes the delivery of the items of the order in the quantities specified in the Price and Tender Forms for the period:

- Part 1 to 2 – 12 months.

5.The Contracting Party shall not allow the submission of equivalent tenders.

Irinotecani hydrochloridum trihydricum i.v. – 5 500 in a unit of measurement of 100 mg.

Detailed description of the object of the contract, requirements of the Ordering Party, information on the acceptance of other quantities in the packaging, are set out in the Price and Tender Form, Annex No 1 to SWZ.

Due to the program (the duration of the order) stated: 12 months should be:

Planned delivery of order items:

- Part No. 1 - for 12 months, counting from the date of conclusion of the contract.

The Ordering Party in Part No. 1 allows the possibility of performing the contract after exhausting the number of current contracts.

The Contracting Authority shall allow the modification of the term of application of the contract concluded with the Contractor in the event that the whole subject of the contract is not fulfilled during its duration. In this case, the amendment of the contract requires the signing of an annex, provided that the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the time limit for which it was concluded.

Vedolizumabum solution for injection, 108 mg/0.68 ml x 2 injection. 0.68 ml – 230 op.

- Part No. 2 - for 12 months, counting from 27.01.2023.

In Part No. 2, the Ordering Party allows the possibility of earlier purchases in the event of exhaustion of the quantity from the current contract. In this case, the term of the contract will be counted from the moment of the first purchase of the given subject of the order.

The Contracting Authority in Part No. 2 allows the possibility of a later commencement of the contract in the event of prolongation of the tender procedure.

The Contracting Authority shall allow the modification of the term of application of the contract concluded with the Contractor in the event that the whole subject of the contract is not fulfilled during its duration. In this case, the amendment of the contract requires the signing of an annex, provided that the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the time limit for which it was concluded.
