1. The subject of the order is the inspection of chimney flues and cleaning of flues in service in residential and commercial buildings as well as own offices administered by the ZGN Praga - Południe Capital City Warsaw AN-1 (part 1), AN-5 (part 2), AN-6 (part 3), AN-7 (part 4) in 2019-2020.

2. The subject of the contract includes, among others:

1) checking all chimney flues (regardless of their type).

2) clean the cables.

3) issuing a chimney sweep opinion,

4) control of chimney flues using a camera

3. The scope of works is determined by: the scope of activities to be performed as part of the inspection and inspection of chimney ducts and cleaning of chimney ducts, as well as the address list of buildings together with the list of chimney ducts constituting the connector No. 6 to the ToR

The Employer requires that the Contractor or the Subcontractor throughout the entire period of performance of the subject of the contract employ under a full-time employment contract at least 1 person (for one part of the contract) providing work related to the performance of the subject of the contract during the implementation of the subject of the contract. This requirement is also considered to be met if more people are employed on a part-time basis, as long as this employment is equivalent to 1 full-time employment and this will not affect the correct performance of the contract.

5. The above condition will be met by employing new employees under the employment contract or appointing employees already employed by the Contractor or Subcontractor to perform the contract.

6. The scope of activities to be performed as part of the inspection and inspection of chimney ducts, cleaning of chimney ducts and the address list of buildings with the list of chimney ducts intended for implementation by persons employed under an employment contract is described in Description of the subject of the order constituting Connector No. 2.

7. The contractor shall, no later than two days after signing the contract, document the form of employment of the employees declared by him in the "List of persons who will participate in the performance of the contract", showing, for example, an employment contract, ZUS RCA printing, etc., which will clearly indicate the number full time.

8. The Employer reserves the right to control the employment of persons indicated by the Contractor to perform the contract. The inspection may be carried out without prior notice to the Contractor.

9. The Contractor may change the persons indicated in the "List of persons who will participate in the performance of the contract". In this case, the Contractor is obliged to notify the Employer in writing. The notification must be accompanied by copies of the same type of entitlements and not less than those required in the public procurement procedure. Lack of required permissions at notification makes the change ineffective. In the event of a change of persons, the Contractor undertakes to provide the Employer with documents confirming employment. This obligation is fulfilled by the Contractor within 3 business days of making the said change.

10. Sanctions for failure to meet the requirements set out in paragraph 1 specifies the contract template constituting Annex 9 to the ToR.
