The scope of the contract includes the following tasks:1.Reconstruction and extension of the former mill with a change of use into a multifunctional hall, museum and café with facilities;2.Renovation of the Kretzschmar Palace of the current USC;3.Construction of the building at Brzozowa Street for the needs of meetings of seniors, associations and NGOs according to the design and build formula;4.Development of squares and city parks as an element of the layout of green areas in Chełm planned in 11 locations according to the design and build formula, 5. Modernization (reconstruction) of the Jordanowski Garden according to the design and build formula;6. Modernization of the SkatePark and Pump Truck located in the city park according to the design and build formula;7.Development of design documentation for the implementation of new investments. Criteria: Price 60%, Warranty period 40%. The proceedings are conducted in accordance with the rules provided for in the reverse procedure referred to in Article 139(1) of the Act.

The investment is intended for co-financing from the Government Fund Polish Governance: Strategic Investment Program.The subject of the contract is defined by: Draft Agreement Provisions (PPU) - Annex 6 to the SWZ, Design documentation and technical specification of execution and acceptance of works (for task 1-2) - Annex No. 9 to the SWZ, Functional and Utility Program (for tasks 3-6) - Annex No. 10 to the SWZ, Description of the subject of the contract (for task 7) - Annex No. 11 to the SWZ. The Contractor may entrust the performance of part of the contract to Subcontractors.Contractors may jointly apply for the award of the contract. In such a case, the Contractors shall appoint a representative to represent them in the procurement procedure or to represent them in the procedure and conclude a public procurement contract. The power of attorney should be attached to the offer in electronic form. In the case of Contractors jointly applying for the award of a contract, the European Single Procurement Document (ESPD) shall be submitted by each of the Economic Operators jointly applying for the contract. This statement initially confirms the fulfilment of the conditions for participation in the proceedings and the absence of grounds for exclusion to the extent that each of the Contractors demonstrates compliance with the conditions for participation in the proceedings and the absence of grounds for exclusion. The Contractor is obliged to submit together with the offer a statement constituting a preliminary confirmation that the Contractor as at the date of submission of tenders:a) is not subject to exclusion,b) meets the conditions for participation in the procedure

The declaration referred to in point 6.1 of the SWZ shall be submitted by the Contractor in the form of a single document drawn up in accordance with the model standard form set out in the European Commission Implementing Regulation 2016/7 of 5 January 2016 issued on the basis of Article 59(2) of Directive 2014/24/EU, hereinafter referred to as the 'Single Document' or 'ESP'. – according to Annex 2 to the SWZ. The contracting authority provides for the grounds for exclusion indicated in Article 109(1)(5) and (7) of the Act. In order to confirm the absence of grounds for exclusion from participation in the proceedings:1) information from the National Criminal Register in the scope of:a) Article 108(1)(1) and (2) of the PPL;b) Article 108(1)(4) of the PPL, concerning the prohibition of applying for a public contract as a penal measure, - drawn up not earlier than 6 months before its submission;2) the Contractor's statement regarding Article 108(1)(5) of the PPL, on the lack of belonging to the same capital group, within the meaning of the Act of 16 February 2007 on competition and consumer protection (Journal of Laws of 2021, item 275, as amended), with another Contractor who submitted a separate offer, or a statement on belonging to the same capital group together with documents or information confirming the preparation of the offer, regardless of another Contractor belonging to the same capital group. - Annex 4 to the SWZ;3) the Contractor's statement on the validity of the information contained in the statement referred to in Article 125(1) of the Act, to the extent relating to the grounds for exclusion indicated in Article 108(1)(3)-(6) of the Act and in the scope of the grounds for exclusion indicated in Article 109(1)(5.7) of the Act. Model - Annex 7 to the SWZ. Exclusion pursuant to Article 7(1) of the Act of 13 April 2022 on special solutions in the field of counteracting support for aggression against Ukraine and serving to protect national security and Exclusion of the Contractor is also based on Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 concerning restrictive measures in connection with Russia's actions destabilising the situation in Ukraine.In the proceedings in question, communication between The Contracting Authority and the Contractor, including the submission of tenders, exchange of information and transfer of documents or statements, taking into account the exceptions provided for in the Act, shall be carried out using electronic means of communication, via the Platform located at the address given above.
