The material scope of the contract includes: 1) Winter maintenance of the road surface, sidewalks (excluding sidewalks directly adjacent to the property), pedestrian crossings, parking lots, bays and bus stops, bicycle paths, green belts of national, provincial and district streets on the

the area of the city of Chełm; 2) Winter maintenance of road surfaces, sidewalks (excluding sidewalks directly adjacent to the property), pedestrian crossings, parking lots, bays and bus stops, bicycle paths, green belts of municipal streets and internal roads in the city of Chełm;

3) Post-winter road cleaning specified in § 2 section 7 PPU; 4) Other cleaning activities specified in § 2 section 8 OF THE PPU.

The order and rules of winter maintenance, lists of streets, sidewalks and areas entrusted for winter maintenance are specified in the provisions of § 2 of the Proposed provisions of the contract and the following documents:1) List of areas to be maintained in the city of Chełm – Appendix No. 1 to the Proposed provisions of the contract;2) List of urban real estate – Annex No. 2 to the Proposed provisions of the contract;3) Instruction for use and maintenance of the national road No. 12 – Appendix No. 3 of the Proposed Provisions Agreement. Criteria for the evaluation of tenders: Price -60 points, Amount of the contractual penalty for delay in removing defects found on receipt 24 points, Time of performance of the "intervention" service from the occurrence of the notification by the Ordering Party - 16 points. Economic operators may jointly apply for the award of a contract. In such a case, the Contractors shall appoint a representative to represent them in the procurement procedure or to represent them in the procedure and to conclude a public procurement contract. The power of attorney should be attached to the offer in electronic form. The Contracting Authority requires that in the case of entrusting part of the contract to Subcontractors, the Contractor indicates in the offer the parts of the contract which it intends to entrust to subcontractors and provide (if they are known to it at this stage) the names (companies) of these Subcontractors.The offer must contain:1)Offer form prepared according to the template of the Offer Form constituting Annex 1 to the SWZ. In the case of submitting a tender on a form other than Annex 1 to the SWZ, it should contain all the required information specified in this Annex.2)Single European Procurement Document (Annex 2 to the SWZ),3)Obligation of another entity to make available the necessary resources to the Contractor (if applicable),4)Calculations of the price of the offer, drawn up according to the template constituting Annex 11 to the SWZ, 5) Statement of exclusion, drawn up according to the template constituting Annex 10 to the SWZ,6)documents from which the right to sign the offer (appropriate powers of attorney) arises,7) power of attorney.
