Primary Medical Services in Derby City.

The practice has a current list size of approx.11 000 patients and is provided from 2 locations:

1. The Lister House — Chellaston Fellow, Lands Way, Chellaston, Derby, DE73 6SW.

2. The Lister House — Allenton Coleman Health Centre, Coleman Street, Allenton, DE24 8NH.

These services must be provided by GPs and Nurses and the provider will be expected to deliver:

— Essential services;

— All additional services;

— Dementia and learning disabilities;

— All enhanced services commissioned by NHS England and SDCCG;

— Opening hours and service delivery from 8:00-18:30, Monday-Friday (except statutory bank and public holidays);

— Increased access;

— Promotion of healthy living with the aim of tackling the underlying causes of ill health;

— High quality primary medical services responsive to the needs of the local population.
