The subject of the contract is: the service of renting, installing and commissioning 22 multifunctional devices in accordance with the specification set out in point 7 of the OPZ together with comprehensive service for three municipal organizational units:
Shared Services Centre – 8 devices,
Road and Transport Authority – 6 devices,
Municipal Investment Board – 8 devices.
Rental, installation, commissioning and technical support of software for managing and monitoring print and follow-up printing for rented devices and devices of the Ordering Party (shown in point 4 of the OPZ) meeting the requirements described in point 8 of the OPZ.
The Ordering Party allows the provision of printing and printing services following the printing system described in point 3 of the OPZ and the use of the license held by the Ordering Party for 20 rented devices.

6. Deadline for the execution of the order – from the date of signing the contract to 31.12.2025.
9.3.1 In order to confirm that the Contractor meets the conditions for participation in the procedure regarding technical or professional ability, the Ordering Party requests the following documents:
9.3.1.1 List of deliveries (according to the model – Annex No. 5 to SWZ) made
within the last three years before the deadline for the submission of tenders, and if the period of activity is shorter, during that period, together with their value, subject matter, dates of performance and entities, to whose supplies have been made and attached evidence specifying whether these supplies have been duly made, the evidence in question being:
-references; or other documents drawn up by the person to whom the supplies have been made; and if the Contractor is unable to obtain these documents for reasons beyond his control – declare. Artist.
9.3.2 In order to demonstrate the absence of grounds for exclusion from the Procurement Procedure of the Contractor, the Contracting Authority requests the following documents:
9.3.2.1 Declare to the Contractors within the scope of Article 108(1)(5) of the PPL Act on the lack of membership in the same capital group within the meaning of the Act of 16 February 2007 on competition and consumer protection (i.e. Journal of Laws of 2021, item 275) with another Contractor who submitted a separate offer, or declare their membership in the same capital group together with documents or information confirming the preparation of the offer independently of another Contractor belonging to the same capital group – model declare. is Annex No. 9 to SWZ.
9.3.2.2 Declare to the Contractor the validity of the information contained in the declaration referred to in:
-Article 125(1) of the PPL Act (i.e. in the JAD) to the extent related to the grounds for exclusion indicated in Article 108(1)(3)-(6) of the PPL Act,
-Article 5k of Regulation 833/2014 as amended by the Regulation. 2022/576 and Article 7(1) of the Act on special arrangements for countering support for aggression against Ukraine and protecting national security. In the case of contractors jointly applying for a contract, the Contractor's statement on the validity of the information contained in the JEDZ is submitted by each of these Contractors (also applies to partners of a civil law partnership).
