In line with the Government's objective to greatly increase the integration of health and social care, night care services in Bassetlaw are commissioned by the Clinical Commissioning Group. A single contract, to be known as 'Care at Night', will enhance continuity of care for individuals. The Care at Night Service is commissioned under the current NHS Standard Contract and is subject to the overarching NHS Standard Contract General Conditions and Service Conditions.
More people in Bassetlaw are being supported to live at home for longer, as demonstrated by a 33% reduction in care home admissions from 2015 to 2018. While this is a positive outcome, an ageing population and other demographic pressures have increased the demand for community-based health and social care services. Night care continues to play a vital role in ensuring people are able to remain independent, safe and well in their own homes.

Care at Night will support people with eligible needs to remain safe and well in their own home during the night. The Service will be flexible and responsive, delivered by a Provider empowered to take proactive action to ensure eligible needs and desired outcomes are met. The responsible Provider will deliver an efficient, flexible service which is responsive to changes in demand and ensures capacity is utilised effectively.
Objectives of Service:
People are supported to ensure the outcomes most important to them are achieved and as a result are highly satisfied with the Service
Frontline workers feel well trained, supported and confident in their ability to provide high quality provision
People are able to return home from hospital within 24 hours if they have chosen to die in their own home
New requests for the Service commence within the required timescales.
