The subject of the contract are the supply of Medical Equipment - Materials for Surgical Procedures for the needs of the Independent Public Health Care Team in Myszków. The order is divided into 29 packages (parts). The delivery specification is contained in Annex 1.1 to the Terms of Contract ("Price Form"). Other terms of the contract are set out in Annex 2 to the tender specifications ("Draft provisions of the public procurement contract to be incorporated into the contract"). Agreements for individual packages will be concluded after the completion of these proceedings for a period of 12 months.
