The subject of the order is the collection of municipal waste collected in containers and bags on residential real estate located in the commune-city of Płock in any quantity:

1) mixed (segregation residues);

2) paper - waste from paper, including cardboard, packaging waste from paper and cardboard;

3) metals - metal waste, including packaging waste from metal; plastics - plastic waste, including plastic packaging waste; multi-material packaging waste;

4) glass - glass waste, including glass packaging waste (white and colored);

5) bio waste (kitchen, resulting from the care of green areas, including Christmas trees (in full));

6) construction and demolition waste constituting municipal waste;

7) furniture and bulky waste (including renovation materials e.g. toilet, windows, doors);

8) hazardous waste and obsolete chemicals;

9) tires from passenger cars.

11) waste batteries and accumulators;

12) expired drugs;

13) waste not qualifying for medical waste generated in the household as a result of receiving medicinal products in the form of injections and monitoring the level of substances in the blood, in particular needles and syringes (also known as medical).

1. The offer should also include:

1) proof of payment of the bid security - if it was lodged in a form other than cash;

2) power of attorney of the person or persons signing the offer - if the right to sign does not arise directly from the attached documents;

3) in the case of a joint bid - a power of attorney granted to the leader;

4) list of vehicles - Annex 2;

5) price form - Annex 3;

6) third party's obligation, if applicable - Annex No. 4.

2. The Employer excludes the Contractor from the contract award procedure if there are grounds for exclusion in relation to him, referred to in art. 24 paragraph 1 points 12-23 and pursuant to Article 24 (5) points 1, 2, 4.
