Provision of a pharmaceutical and medicines support service to patients in their own home will include the following:
— pharmaceutical assessments of person referred and reassessments and review by the Service Manager and designated assessors,
— on-going support to eligible patients through the provision of an appropriate intervention by pharmacists,
— administration including payment of participating pharmacists,
— training, support and advice in medicine management to health and social care professionals and care providers.
Provision of a pharmaceutical and medicines support service to patients in their own home will include the following:

— training, support and advice in medicine management to health and social care professionals and care providers supporting people to remain independent at home for longer and be less reliant on health and social care services through medication concordance resulting in improved treatment outcomes and quality of life,
— to support patients with medicine related problems to ensure their compliance and concordance with taking their medication,
— to identify any medicine related risks,
— to optimise medication regimes, thereby improving patient safety and outcomes and reducing medicine wastage and hoarding,
— to support Domiciliary Care Providers accredited with NCC to comply with Norfolk Medicines Management policy.
