Provision of services for transporting people for hemodialysis performed in the Dialysis Center of the Clinical Dialysis of the Provincial Hospital No. 2 St. Jadwiga Królowej in Rzeszów

1. The subject of the contract is the service of transporting people for hemodialysis performed at the Dialysis Center of the Clinical Dialysis Center of the Provincial Hospital No. 2 St. Jadwiga Królowej, i.e. transport for treatments at 60 Lwowska Street in Rzeszów and transport after hemodialysis to the indicated address of residence. A detailed description of the subject of the contract is provided in:

a) Annex 4 to the SWZ (OPZ)

b) Annex 3 to the SWZ (Draft Agreement)
