Construction of a school and kindergarten team at ul. Anna German in Warsaw "- Stage I
The subject of the order in the first stage is: construction of an educational building containing a primary school, kindergarten and library together with installations, including:
- construction of technical infrastructure: water connections, sanitary sewage and power lines, area lighting,
- land remediation.
A four-storey building with a basement, a rectangular shape with an internal patio was designed. On the north side there is a gym from the west, the L-shaped kindergarten part. The building is designed for approx. 800 children in primary school and for 150 children in kindergarten. About 150 people are expected to be employed.
In connection with the application of the social clause based on art. 29 paragraph 3a of the Act of 29.1.2004 Public Procurement Law (Journal of Laws of 2017, item 1579, as amended). Employer:
a) requires employment by the Contractor or Subcontractor, based on an employment contract, 15 employees providing work related to performing activities at general construction works, i.e. earthworks, construction and building, construction and finishing works, related to sanitary and electrical installations
Information clause from art. 13 GDPR
In accordance with art. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (General Regulation on data protection) (Official Journal EU L 119 of 4.5.2016, p. 1), hereinafter referred to as GDPR, I inform that:
1) the administrator of your personal data processed in the City Hall Warsaw is: President of the Capital City of Warsaw Warsaw, pl. Bankowy 3/5, 00-950 Warsaw, POLAND;
2) a personal data protection inspector at the City Hall Warsaw is the Data Protection Inspector at the City Hall Warsaw, contact: iod@um.warszawa.pl;
3) Your personal data will be processed on the basis of art. 6 clause 1 lit. c GDPR for the purpose related to the procedure for awarding a public contract, reference number UD-XVIII-WZP.271.73.2018, conducted in the form of an open tender;
4) recipients of your personal data will be persons or entities to whom documentation of proceedings based on art. 8 and art. 96 section 3 of the Public Procurement Law;
5) Your personal data will be stored in accordance with art. 97 paragraph 1 of the Public Procurement Law, for a period of 4 years from the date of termination of the contract award procedure, and if the duration of the contract exceeds 4 years, the storage period covers the entire duration of the contract;
6) the obligation for you to provide personal data directly related to you is a statutory requirement specified in the provisions of the Public Procurement Law related to participation in the public procurement procedure; the consequences of not providing specific data result from the Public Procurement Law;
7) with regard to your personal data, decisions will not be taken in an automated manner, applying to art. 22 GDPR;
8) you have:
a) pursuant to art. 15 GDPR, the right to access your personal data;
b) pursuant to art. 16 GDPR, the right to rectify your personal data;
c) pursuant to art. 18 GDPR, the right to request the administrator to limit the processing of personal data, subject to the cases referred to in art. 18 clause 2 GDPR;
d) the right to lodge a complaint to the President of the Office for Personal Data Protection, if you feel that the processing of your personal data violates the provisions of the GDPR;
9) you are not entitled to:
a) in connection with art. 17 clause 3 lit. b, d or e RODO the right to delete personal data;
b) the right to transfer personal data referred to in art. 20 GDPR;
c) pursuant to art. 21 GDPR, the right to object to the processing of personal data, because the legal basis for the processing of your personal data is art. 6 clause 1 lit. c GDPR.
